Union Community Care Pop Up Primary Care Team

Lancaster County Chooses Love is proud to partner with Union Community Care to host its Pop Up Primary Care Team at our Resource Center.

The Pop Up Primary Care Team is like a Union Community Care exam room on wheels. A mobile care team provides convenient, affordable, and personalized healthcare directly in the community. This program is designed to help remove barriers that may make it difficult to access care, including transportation, childcare needs, and work schedules.

Services Available

The care team can provide a wide range of primary and preventive healthcare services, including:

• Immunizations
• STI testing
• Physicals for children and adults
• Lab work
• Help connecting to mental health, pregnancy, dental, and other services
• Additional primary and preventive care services

Who Can Receive Care?

The Pop Up Primary Care Team is available to anyone who needs healthcare services or help finding a regular healthcare provider.

New and established patients are welcome. Community members can receive care with or without insurance, and patients may also continue receiving services at other Union Community Care locations.
