A mobile care team provides convenient\, affordable\, and personalized healthcare directly in the community. This program is designed to help remove barriers that may make it difficult to access care\, including transportation\, childcare needs\, and work schedules. \n\n\n\nServices Available\n\n\n\nThe care team can provide a wide range of primary and preventive healthcare services\, including: \n\n\n\n• Immunizations• STI testing• Physicals for children and adults• Lab work• Help connecting to mental health\, pregnancy\, dental\, and other services• Additional primary and preventive care services \n\n\n\nWho Can Receive Care?\n\n\n\nThe Pop Up Primary Care Team is available to anyone who needs healthcare services or help finding a regular healthcare provider. \n\n\n\nNew and established patients are welcome.
